Agreement

These Terms of Use (“Terms”) govern your access to the Southern Virtual Health (“SVH,” “we,” “us”) website, patient portal, eligibility questionnaire, and telehealth services. By using this site or requesting care, you agree to these Terms, our Membership Agreement, Financial Agreement and Cancellation Policy, our Privacy Policy and HIPAA Notice, and our Arbitration Agreement.

Who we serve

SVH provides telehealth only to patients who reside in Alabama and who will remain in Alabama for the duration of treatment. You must be physically located in Alabama at the time of each telehealth visit. Care is provided by Alabama-licensed physicians and nurse practitioners, with support from Alabama-based staff. If you are not located in Alabama at the time of a visit, we cannot treat you.

Not emergency care

This website and our services are not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency department. Eligibility screening, emails, and portal messages are not monitored in real time for urgent symptoms.

Eligibility is not treatment

Passing the eligibility questionnaire and creating a portal account do not guarantee that you will be accepted as a patient or that any medication will be prescribed. Treatment decisions are made only after a licensed clinician completes a clinical evaluation. Prescriptions, if appropriate, are sent to the manufacturer (Eli Lilly / LillyDirect or Novo Nordisk / NovoCare). You purchase medication from the manufacturer. Membership does not include the cost of medication.

Membership and payment

SVH is cash-pay. Membership is billed monthly. We do not bill insurance. Optional dietitian and pharmacist services are extra. Extra visits beyond the standard schedule are cash-pay. Fees are non-refundable except as required by law or as we agree in writing.

Your responsibilities

• Provide accurate identity, residency, history, and lab information.
• Upload required documents and keep your portal information current.
• Follow visit instructions and disclose other medications and conditions.
• Use medications only as prescribed and store them as directed.

Limitation of liability

To the fullest extent permitted by Alabama law, SVH and its clinicians and staff are not liable for indirect, incidental, or consequential damages arising from use of the site or services. Some limitations may not apply where prohibited.

Governing law

These Terms are governed by the laws of the State of Alabama, without regard to conflict-of-law rules. Disputes are resolved as described in the Arbitration Agreement.

Changes

We may update these Terms. The “Last updated” date will change when we do. Continued use of the site after an update means you accept the revised Terms.
