The $175 monthly membership is cash-pay clinical care with Alabama physicians, CRNPs, and support staff. It covers scheduled visits — the initial intake in month 1, a month-2 follow-up if medication is prescribed, then visits every 3 months — plus ongoing education and team support. It does not include the cash-pay cost of medication. If a clinician decides a prescription is appropriate, brand-name Wegovy, Zepbound, or Foundayo is sent to LillyDirect or NovoCare. You pay the manufacturer. They ship to your Alabama address. Optional dietitian and pharmacist support is extra. We do not bill insurance.
Passing the website screening quiz does not waive the $175 charge for the initial visit. That visit is a paid clinical evaluation. If the prescriber determines you are not a candidate to start brand-name medication, the $175 for that visit remains due and is non-refundable except as required by law. Ongoing visits and membership for treatment would not continue.

Comprehensive evaluation of your medical history, current health, weight-management goals, and eligibility for treatment. If appropriate, a brand-name GLP-1 or GIP medication is prescribed and coordinated through manufacturer cash-pay programs.
One-month follow-up
Review of medication tolerance, side effects, early progress, and any needed adjustments. Focus is on safety, adherence, and questions as your body adjusts.
Ongoing visits (every 3 months)
Continued monitoring of progress, medication response, and overall health. These visits include clinical review, support for sustainable habits, and plan adjustments as needed. If medication remains appropriate, your next prescription is written at these visits and coordinated through the manufacturer. Additional visits can be scheduled sooner if clinically indicated.
Optional additional visits

Passing the eligibility questionnaire and completing intake do not guarantee that you will be accepted as a patient, that treatment will continue, or that a medication will be prescribed. Eligibility for care is determined only after clinical review by our physicians and nurse practitioners, in their independent professional judgment. The initial visit is a paid clinical evaluation. Payment for that visit is due even if you are not deemed eligible for ongoing treatment or a prescription, and is non-refundable except as required by law.
