Membership Agreement

What membership is

Southern Virtual Health (“SVH”) membership is a cash-pay arrangement for telehealth visits and related clinical support. It is not insurance. It does not include the cost of medication. Optional dietitian and pharmacist services are billed separately if you add them.

Who may join

Membership is available only to adults who reside in Alabama and who will remain in Alabama for the duration of treatment. Care is provided by Alabama-licensed physicians and nurse practitioners. Passing the eligibility questionnaire and completing intake do not create a membership or a right to treatment. A licensed clinician must accept you as a patient after clinical review.

Visits

The standard schedule is an initial visit, a visit at one month, then visits every three months, or sooner if clinically indicated. Extra visits are available as cash-pay and are not included unless scheduled and paid. Missed visits without timely notice may be treated as a completed visit.

Prescriptions

If a clinician decides a brand-name medication is appropriate (Wegovy, Zepbound, or Foundayo), the prescription is sent to the manufacturer (Eli Lilly / LillyDirect or Novo Nordisk / NovoCare). You purchase directly from the manufacturer. No compounding. SVH does not guarantee that a medication will be prescribed or that a manufacturer will fill it.

Not emergency care

Membership is not emergency care, urgent care, or 24-hour monitoring. If you have an emergency, call 911 or go to the nearest emergency department.
