Privacy Policy / HIPAA Notice

Scope

This Privacy Policy describes how Southern Virtual Health (“SVH,” “we,” “us”) collects, uses, and shares information when you visit our website, complete eligibility questions, create a portal account, or receive care. Protected health information is also covered by our Notice of Privacy Practices below.

Information we collect

  • Contact details such as name and email.
  • Eligibility answers, including age, height, weight, BMI, and health history.
  • Portal records: agreements, history intake, identification, Alabama residency proof, and labs.
  • Visit notes, prescriptions, and membership or add-on payment records.
  • Technical data such as browser type and pages visited.

How we use it

We use information to screen eligibility, provide telehealth, send follow-up instructions, coordinate manufacturer prescriptions, operate the patient portal and community forum, process cash-pay membership, and meet legal and professional requirements.

How we share it

We share information only as needed: with Alabama clinicians and staff on your care team; with Eli Lilly or Novo Nordisk when a prescription is sent to the manufacturer; with vendors who host our site, email, or records under confidentiality terms; and when required by law. We do not sell your personal information.

Cookies and email

The site may use essential cookies to keep you signed in. If you submit your email after eligibility, we send instructions about next steps. You may ask us to stop non-clinical marketing emails; we may still send messages needed for your care.

Security and retention

We use reasonable administrative, technical, and physical safeguards. No electronic system is perfectly secure. We keep records as required by Alabama medical-record and billing rules, then dispose of them appropriately.

Notice of Privacy Practices (HIPAA)

If you become a patient, federal law protects your health information. We may use and disclose protected health information to treat you, obtain payment for permitted services, and run our practice. We may also disclose information when required by law, for public health, or to avert a serious threat. You have the right to access and request amendments to your records, request restrictions, request confidential communications, and receive an accounting of certain disclosures. You may file a complaint with SVH or the U.S. Department of Health and Human Services. We will not retaliate for a complaint.
