Financial Agreement & Cancellation Policy

Cash-pay only

Southern Virtual Health does not bill insurance, Medicare, or Medicaid for membership. You are responsible for all charges. Medication is purchased from the manufacturer and is not included in membership.

Membership fee

Membership is $175 per month unless we state a different amount in writing. Optional dietitian and pharmacist add-ons and extra visits are additional cash-pay charges. Add-on pricing is provided upon inquiry at support@southernvirtualhealth.com.

Initial visit

The initial visit is a paid clinical evaluation. Payment is due even if, after review by our physicians or nurse practitioners, you are not accepted for ongoing treatment or a prescription is not written. Passing screening questions and completing intake do not create a right to a refund.

Refunds

Fees are non-refundable except as required by Alabama or federal law, or if we agree in writing. Manufacturer medication charges are between you and Eli Lilly or Novo Nordisk.

Cancellation

You may cancel future membership billing by written notice through the patient portal or to support@southernvirtualhealth.com. Cancellation stops upcoming monthly charges after the current paid period. It does not refund amounts already paid for visits or the current month, except as required by law. We may end membership if you are no longer in Alabama, if treatment is not appropriate, or if you do not follow these policies.
